Dr. DeClerk has been my PCP since I moved to the area in 2007, and I cannot say enough about the level of patient care, compassion, and overall customer service he and his entire team bring to the table. No matter the issue I always leave my appointment feeling heard and cared for. Mostly I love the relationships that have developed over time. My husband and son also see Dr. DeClerk for their Primary Care needs and rave about him. In an area like NWA that is experiencing rapid growth, I love the feeling of small town community that has remained at the core of their values! Highly recommend!

Dr DeClerk is great! His billing department helps when insurance isn’t cooperating, he answers questions in a timely manner, and is very attentive to your needs.

So easy to get a same day appointment. Dr. DeClerk is always so attentive and is always tries to give you options for treatment. The staff is very nice and there is never a long wait.

Dr. DeClerk truly cares about his patients. He takes the time to answer any questions I’ve ever had, and gives me multiple options on how to attack problems, instead of just throwing pills at it to see what sticks. If anyone is looking for a physician who earnestly wants the best for you, I would highly recommend giving him a shot.

Wellness visit: was checked in by Paige, the nurse. She took all the vitals and reviewed my chart and history. Dr DeClerk followed up we discussed my current health issues.
