Disappointed in the agency and physician(s) referring. My Mother was involved in an accident and is needing medical care for fractures in both legs. She is non-weight bearing in the left leg and weight bearing as tolerated on the right but has to wear a boot. She is of advanced age (she wouldn't want me to say her age) and has diabetes and potential complications that she may be encountering with managing her blood sugar during this ordeal. I live out of state and hear her telling me about the difficulty she is having getting into the bathroom, transfer from bed to W/C, that she feels very unstable trying to keep her left leg off the ground during these transfers and at significant risk for putting weight on her left leg and hindering the healing of the fracture.
I've put calls into her Ortho Dr and PCP to express my concerns about her safety with transfers and ability to maintain her non weight bearing status on the left leg. Both offices left it up to me to find a home health agency that would be willing to take her. Ortho Dr stated that it would be hard to find an agency that would take the Auto insurance as the payer. The PCP said an agency wouldn't get paid if she went to live somewhere else and they only had a few visit. She was considering going to stay out of town where she had some more help but really preferred to stay in her home. Ultimately, she planned on staying home. I contacted Integris to see if they would accept the auto insurance as the payer, they said they would. I let the PCP's office know they would accept her (had to leave a message) and that it was not going through her Medicare (wouldn't be a LUPA if she did leave sooner). 24 hours went by and no call, I call Integris and they had not received the referral. Left another message with PCP, and called ortho dr to get a referral to initiate home health PT for transfer/gait training and fall prevention. The next morning, it appears that the referral was sent and Integris contacted my mother but asked her what she wanted them to do for her, and we thought you were leaving in a few days. My mom being very frustrated after spending 3 hours on the floor during the night after falling coming out of the bathroom, got the impression that they really did not want to service her so she got frustrated and told them to forget it. I'm not sure where the disconnect was, but very disappointed in how this agency handled this referral. It may have been due to the lack of information or misinformation that they received from the Dr's office but if you have a referral, let the assessing clinician make the determination of the deficits, impairments, functional limitations, safety concerns, and barriers to healing and set up a Plan of Care with the patient at the center. Not concerns about a LUPA which was not applicable.
