I just got approved today after a month of applying for my CLS license. I was consistent of my follow-ups and I'm glad that the customer service reps that I encountered were polite and helpful. It's not true that you are going to repay again if they are still reviewing your app and the 60 days has already passed they made sure of that, but I'm really grateful that they approved my CLS license today!

After reading all the reviews, I was a bit worried that I won't have a good experience and would have to wait a few months for a response but I was wrong! I received an email a couple days after applying for my CPT license saying that I'm missing some documents (my transcript was en route so it would take awhile). After they received my transcript, I received an email 3 weeks later saying my application was approved and I finally got my license! Major improvements must have been made in the past year since I had a pleasant experience with LFS.

I had the pleasure of speaking to Tonya and she was very helpful… gave me updates and a timeline of when to expect my license…fingers crossed!

The lab hired one phlebotomist for the state of California. Thanks to the acting lab director for his approval of my application, I received my certification as a phlebotomist. It took 7 months to process my certification but it was well worth it.

After trying for months, I recieved an email saying my application had been abandoned. I could pay again, and resubmitt all the documents. NOT TRUE! I contacted my senators office, Senator Brian Dahle by email. I received a responce the same day from Juliane Castillo a member of his staff. They contacted LFS and worked with Armando Tiong Examiner ll. I recieved my license today, no charge. The whole process took less than two weeks. Don't give up, keep all your documentation, and do not forget to vote.

From the previous reviews and my current experience, I think this institution has improved from before but still has a long way to go. Email responses are hit and miss. Sometimes it responded straight away sometimes no response whatsoever. Calling them is hit and miss too. If you are lucky to get connected they can be helpful. But most of the time no one is answering the phone (wait time 40min+). For new applicants, I strongly suggest waiting for all your documents to be received by them before actually submitting your application fee. It takes only 2 days for them to tell you what documents you are missing after submitting the CLS application, but takes forever (3-4 weeks) before they let you know if they receive more documents from you. I understand they are short-staffed but still, it is really frustrating.

If you have everything submit and see what youre lacking. Call/ email every other day to get updates. Dont even bother expediting any documents, it doesn’t go straight to them, they still wait for the state mailroom to sort it and deliver it to them. For this big state, I strongly recommend two more offices, one in central and of course in Southern California. There are too many people trying to apply for licenses at one office in the Bay area to handle inquiries.

This is unacceptable there has been an obvious case of neglect to rob me of my money. Any answers I try to get is met with several days of waiting for a reply. Any deficiency I had I'd fix but over time they took longer and longer to reply. To the point where they obviously did not look into the documents I sent in that were fixed. Getting in contact with whomever is in charge of your application is impossible, the people you call at their office aren't the ones dealing with applications. If anyone has any tips for me to file a proper complaint and/or get my state license elsewhere, please let me know.

I was told my work experience would be credited towards my California CLS license. Applied and paid 280 dollars and submitted all my documents (additional fees) and then was told it was not acceptable by another examiner. Now I lost money and still don’t have my license. I’m really concern with the professionalism and management of this state department.

Quite possibly the most inept, slow moving,
and useless government division. They really should win an award! If you don't want your emails read, phone calls answered, or your applications to be processed smoothly, please use this organization! If you are applying for a license your first time, buckle up. You're 100% going to have a terrible time.

WORST SERVICE! The representative was obnoxious while i was asking about a follow up about my license. I was frustrated about an email saying they havent received my evaluation when i have sent it months ago. I called them and said that the evaluation might not have been accepted because it's not an evaluation from the credited members but IT WAS and is recommended by my school. Talk about my classmate who did the same but hers went smoothly. I dont know how mine is different when we literally did the same thing?

This is the most horrible organization in History I applied for my Phlebotomy Licence over year. I paid 3 times for application process still abounded. No one answered a phone if they did they will not direct you what is missing for a person have transcript out of USA. They ask you to Evaluate non US transcript and I did three times the website they give me and still they said they are not approved the Website where I do my Evaluation. At this point I don’t know what to do. This is horrible company!!!

2 years and still no phlebotomy program approval.
Senator's office, can't believe the fabrication, lack of assistance and our 2 year process waiting.
Unprofessionall staffing. Lack of communication, accountability, follow through, and total disregard for the schools. Jason Boatman and Linda Wraxall should not be employed at this facility. Who is minding the staff and the process. No one!

NO stars!! This is the most frustrating service I've received here in the U.S. Terribly slow, vague email- communication, unhelpful customer care desk. It almost seems like they deliberately drag so they can send you an abandonment notice and request for a new application fee.

I have never felt so defeated in my life trying to communicate with Laboratory Field Services. I am a phlebotomy applicant who had to wait 15 weeks to get a single reply from them only to find out that I was missing an important document to complete my application (Average review takes about 10 weeks). Once I discovered this, I immediately sent them in and am currently waiting for their next “move”. My application is about to expire this month now because of how long everything took and it’s so disappointing. I really hope LFS can figure their stuff out because whatever their system is it’s apparently not working. My $100 application will not be payed for a second time, that’s for sure.

Applied for my CLS training license in November of 2017. March 2018 i get an email saying I am missing documentation and that my application will be abondoned if I do not provide the proper documentation (transcripts, etc.) I mailed everything again, FOR THE SECOND TIME. Today, May of 2018, I get an email saying my application has been abandoned because I failed to respond with the proper docs. I even sent them copy of the mailing receipts. And like everyone here said, it is impossible to reach anybody with any form of communication.

2 years of frustration, no accountability, lack of communication and unprofessionalism by several staff members. Reaching out Senator Brian Dahle and others for assistance, including Channel 3 news.

Let me just start by saying if i could give negative starts... I WOULD! This company is very unprofessional I feel very frustrated. I’m starting to wonder if they even have employees. I’ve sent over 10 emails and never get an answer, I only get random emails of them asking for things that have already been sent and acknowledged receipt by them. To top it off when I call, anyone who answers the phone has no answers to any questions and has no idea what they’re doing. Oh yeah they also loose your paper work and say its in their mailing room and thats why nothing has been processed. But my paperwork has been in the “mailing room” for weeks and I still have no update as to when they are going to start processing my application. I’ve lost my patience, I am upset I didn’t look into this company before because ALL of their reviews are horrible!

This place is insane! They’re putting my career at stake by not responding to calls or emails for over a month. There seems to only be one phone number, and I have gone out of my way to call other CADPH in different counties, city halls, other government departments to see if anyone can reach them! If I could give negative starts, I would! I started calling MONTHS before my license expires... and the time is creeping up with no help! I finally got a contact thru email, and I begged the person to call me, as my inquiry would only take less than 10 min... here we are... 2 weeks later, and no response to my email. Idk what poorly employed circus is being ran in there, but no amount of “ we’re busy” can ever possibly justify this terribleness! Top grade trash.

The LFS need to improve their communication and response to applicants in an efficient manner. I applied for the correct license and was told by LFS I was not applying for the correct license. I was instructed to reapply for the correct license and repay the fee. I did as instructed. A week later, I was issued a license for what they said was incorrect. I requested a refund for this additional application fee I submitted. This is where the process breakdown. I constantly received periodic emails from LFS stating I’m missing documents and my application will be abandoned. I keep responding that I would like a refund because I was granted a license and I was told by an LFS agent to reapply and repay. I have not received an appropriate response yet. Please provide better assistance and customer support to professionals trying to get their state license.
Thank you.
