Everyone is friendly, in and out was convenient, and all team has new equipment in giving me a thorough check up and cleaning and everything I need! The staff were very nice and warm. Wait time was very short! I fell very comfortably with any treatment I have to do here!

The office members really make you feel comfortable. The office is super clean & they’re very friendly & I didn’t even have to wait long. Tahyana at the front is so professional and helpful as well as the doctor Jack & hygienist Bryce. Dana was super nice as well & fast! They definitely made the dentist not as bad as going to the dentist can be!! Highly recommend :)

Deerfield Modern Dentistry has been amazing. I had to have a couple emergency procedures done and they were able to immediately help. I recently needed a deep clean done and Bryce was amazing- he walked me through the whole procedure and was gentle. Everyone is so pleasant to deal with. I have been to Deerfield Modern Dentistry twice, and both times were fantastic! Dr. Jakubowski treated me with great care and quality. The staff is incredible, and it appears that they all genuinely love working together. This office has great chemistry! They prioritize customer satisfaction, and I have to say this was my best experience at a dentist office. All the equipment/machinery looked to be brand new as well. My teeth, gums, and smile are the best they have ever been. I have way more confidence when I smile now that I didn’t have before. The dentist assistant, Lupe, was also amazing. She took very good care of me and is really nice. Thank you again for the great experience!

Dr. Azadi, Lupe & the ladies at the front desk were all just so kind and caring! They helped me so much to be able to work out a payment plan to get my teeth fixed after so many years of struggling to take care of them. I will definitely be back ❤

Brice did a great job cleaning my teeth, I was in and out and under 30 minutes. The facility is clean and modern and the staff is amazing.

I had total mouth extraction and oral surgery. Dr. Azadi performed it. He did a great job. Him and his nurse made me feel calm. There was very little pain; just some uncomfort. I had a follow up with Dr. Cody Craig. He too is awesome! I'd highly recommend this place to anyone. Modern equipment; office perfectly clean, front staff is friendly and efficient.

Great experience here - just moved back to this area and they will be my new dentist home. The receptionist is helpful and friendly, as are the dental assistants, the dental hygienist and Dr. Craig. Everyone was very professional, helpful, thorough, and Dr. Craig is warm, patient, and knowledgeable. I appreciated his professional and kind bedside manner. Both he and the dental hygienist showed they cared about my health and took time to get to know me.

I would definitely recommend Deerfield Modern Dentistry. Cody and Bryce are excellent at their profession. They are knowledgeable, professional, efficient at their job. They explain the procedure before they do it and keep you informed during the procedure while making sure that you are comfortable. I couldn't ask to be treated any better than that. I absolutely love this dentist office. Dr Craig is awesome and made me the most beautiful crown I’ve honestly ever had. The front desk girls are very sweet and professional. Jaelyn deep cleaned my teeth and she was great. I have nothing negative to say about this office.

The office is spotless, the staff is very friendly and knowledgeable, and they offer treatment plans to keep things affordable. My oral hygienist Bryce did a great job explaining at home care. So happy to have discovered this office.

I had a bad experience with a wisdom tooth removal that had scared me away from the dentist office before but I knew I had to get a cleaning done just to check out and be sure that nothing else in my mouth was messed up. During my visit here I ended up getting two cavities filled and the process was amazingly smooth. Everyone is extremely nice and informative as well. 10/10 highly recommend.

This office is amazing. The professionalism, caring attitude, and amazing atmosphere is what had me on the first day. There is transparency and understanding. I would recommend this facility To anyone and everyone looking for a great dental experience.

Dr. Craig not only identified the tooth that was causing the pain when no other doctor could. But also saved my tooth when other wanted to extract and replace with implant. He a brilliant young doctor, utilizing new technology to help his patients. Ashley, is as wonderful as Dr. Craig. Thank you again both for saving my smile.

I had a fantastic experience at Deerfield Modern Dentistry. Upon arrival, Tahyana was super lovely in greeting me and helping with all my forms and financial questions. The office is spotless and very welcoming. The staff all seemed to get along really well which made for a superb environment. I saw Dr Jakubowski for my treatment. Dr Jakubowski went above and beyond compared to every other dentist I’ve experienced. He took meticulous care in every procedure and constantly checked up on me to make sure I was okay. He explained everything he was doing clearly and in great detail so I felt extremely reassured as to what was going on.

Competent, efficient and friendly. Clean, bright and modern as the name suggests. Highly recommend. Tahyana is great if you need a payment plan she is great to work with, you have no excuse for not getting what you need to have done to your teeth.
