just retiring and moving to Lebanon I chose this office from there reviews, I was not dissapointed. I was greeted by a efficient and friendly front desk filling out new patient paperwork was quick and I was in a chair with friendly dental assistants who made me comforatable thru the entire process. The dentist was friendly gentle and made a root canal a pain free expierience. Thank You! My new Dental Office!

Forum has worked on my mouth for 2 years now. They take really good care of my teeth. They are all so friendly and nice why you are there.

The dental hygienist was very friendly. She was kind to my gums while cleaning my teeth. I also like the dentist because she is a happy person. She’s very positive and doesn’t try to correct all the issues with my mouth.

Went in today to get my dentures relined 5/16/23

My experience was great from the front office staff, the assistant, Dr and the hygienist. They made me feel right at home and the routine of my new patient visit flowed perfectly.

I have had a lot of work done here with crowns and other services. The Dr. here is wonderful he did a great job! Excited to get my bridge work done and more!

My first appointment today at Forum Dental was a pleasant experience....The team was friendly and very knowledgeable. I appreciated the way they explained the procedures and showed me
pictures of my teeth and made a plan for me to get back to where I was before Covid. I would recommend Forum to everyone.

Staff is very nice pulled a Moller and put bone Graf I have been comfortable going to the Dentist
