I had been trying to obtain certain documents for two months. With one phone call I was able to find out how and where to obtain them. It really helped me out!

Rude staff. Neglectful Dr.s. Supervisors that laugh and mock you. Benzodoazine medication not being signed off by Dr. Borja and left sitting in the pharmacy for 3 to 4 days while nurses are trying to email him to fill it. Meds always late, meds sometimes short. A personal note. I have been on Xanax .For three years. I've never ran out and I've only flipped out when they are days late. A month ago I had a fresh prescription that was stolen. I had to call the police as much as I hated to and I got my full refill. My next visit Dr. Borja puts me in the care or Dr. Michael Levy who takes me off Xanax and puts me on librfium for2 weeks. After returning for two weeks he told me I could go back on Xanax or stay off benzos all together. Of course I declined seeing it's my most helpful medication. He then puts me on 1 mg Ativan and tells me it's eqaulviant to my 1mg Xanax. A simple Google search will tell you 1mg of Ativan is eqaulviant to 0.5 of Xanax. I went into witjdraw from actually taking this medication. Zero calming effect. I had also told Dr. Levy that I've been on lorazepam in my past and it's side effects sent me into a rage and mania. I didn't know lorazepam and Ativan were the same drug. I call the health department explaining the situation and he recommends that I go to a three day detox because he's cutting me off of benzos entirely. Almost 3 yeat's on benzos then Michael Levy puts me on a medicine I told him treated me terribly. I refuse to go to a detox clinic. Guess I'll have to check out street prices.

My mom needed her birth certificate in order to get a new Id card. so as directed we went here to get it. Told us we need a valid ID card to get her birth certificate. Receptionist was rude; had to explain we don't have one that's why we were there.

Particular lady was very loud and had the TV on during the whole evaluation process. Was very judgemental about certain types of things we were discussing.
